Enrolling in Boost Protect is easy! Simply sign up when you buy your new phone or device and your coverage starts immediately.

Need a little time to think it over? Enrollment is available for 30 days after you purchase your phone, so be sure to act quickly! Please note that your device must be fully operational at the time of program activation in order to enroll.

Low Monthly Premium A fee of $7 will be added to your monthly Boost Mobile service charge.

Affordable Deductibles A $20, $50, $100 or $175 deductible will apply depending on your phone. Your deductible is established at the time of purchase and is based on the non-subsidized price of the make/model of your device. Once enrolled, your deductible does not change even if the store price of your device decreases or increases.

You are allowed 2 approved claims in any rolling 12-month period beginning with the date of your first approved claim.

What does my program cover?

How do I cancel coverage?

Contact Boost Mobile directly at 833-50 Boost (833-502-6678). They will help you change or cancel coverage.

What should I do with my damaged device?

If your phone was accidentally damaged and your claim is approved, you’ll need to ship us your broken device. Shipping instructions and packing materials for returning the claimed equipment will be included with the replacement device we send you. Be sure to act quickly! Failure to do so within 10 days may result in equipment and processing fees of up to $200.

Avoid a locked device fee by disabling your Find My iPhone app and removing the device from your iTunes account. Learn more.

How many claims am I allowed?

You can make up to two claims per rolling 12-month period.

When do I file a claim?

If your phone is lost or stolen and you have activated McAfee Mobile Security app, you can attempt to locate your phone here.

If your phone or device is lost, stolen, or damaged, file a claim.

You can also file a claim if your phone or device experiences an electrical or mechanical malfunction and more than 12 months has passed since the date of purchase.

A manufacturer's warranty (in-warranty) is provided by your phone manufacturer and is generally valid for 12 months from the date of purchase. Electrical or mechanical malfunctions in these first 12 months that are no cause of your own are typically covered by your manufacturer’s warranty. Details can be found in your original packaging, but can also be obtained at your carrier location.

How do I file a claim?

Our online claims process is fast, easy, and secure. Simply gather your phone and personal information, file your claim online, and keep an eye out for your replacement phone.

What do I do if I have issues uploading my documentation to My Plan?

If you are experiencing issues uploading your claim affidavit or other requested documents, you can email documents@Likewize.com or you can send by mail to Likewize Device Protection, LLC Attn: Asset Protection, 1900 W Kirkwood Blvd, Suite 1600C, Southlake, TX, 76092, USA. Requested documentation sent by mail or email may be delayed.

Will I get the same phone for an approved claim?

Your replacement phone will be a reconditioned one of like kind and quality. While reconditioned equipment functions just like new, it may have minor cosmetic flaws and contain non-original manufacturer parts and accessories. If the exact make and model is not available, your replacement will be of like kind and quality.

I filed a claim and received a coupon to purchase a new phone. How do I use my coupon?

You can use your coupon in any Boost Mobile store to purchase a new phone and enroll in our new Boost Protect program. You can also use it to purchase anything sold in the store — even wireless service. You should have received an email with your coupon information. Simply go to any Boost Mobile store and provide them with the mobile phone number associated with your claim.

Then choose your new device. Keep in mind that you can only use the coupon in a Boost Mobile store, it has to be used in one purchase, and any unused amount will be lost. You can also find your coupon amount and instructions by logging in to My Plan and going to Claim Details. Your coupon will be activated and ready for use within one business day after you complete your claim.

What do I do if my phone is lost or stolen?

If your claim is for a lost or stolen phone, report the loss or theft to Boost Mobile at 1-888-266-7848 as soon as possible so they can suspend your service, as you will be responsible for any further network charges. Please make sure you ask Boost to block the SIM card and the phone in order to prevent both from being used by anyone across all US networks. Likewize will blacklist your phone’s IMEI to prevent it from being used.

Once this is complete, contact Likewize to file a claim. Claims must be reported within 60 days of the incident or loss.

What security features are included with my Boost Shield program?

McAfee® Mobile Security guards your smartphone against viruses, malware, phishing, and other mobile threats. It also allows you to locate, lock, and wipe your personal data from a lost or stolen phone, and provides 2GB of backup for your contacts, photos and videos.

I'm having problems using the features within my McAfee Mobile Security app. What should I do?

How much does it cost?

A Monthly Fee ranging from $2.75 to $18.00 depending on your device tier will appear on your Boost Mobile bill.

Please note that a deductible is also required when your claim is approved. Repair deductibles range from $29.00 to $99.00 depending on your device and damage type.

Loss and Theft deductibles range from $40.00 to $349.00 depending on your device tier.

If your protected device is returned with the locking feature enabled, you will be charged a Locked Device Fee of up to $800.00 depending on your device tier.

The Claim Conversion Fee is the difference between the phone screen repair deductible and all other damage repair deductible, in instances where there is additional damage beyond screen damage. Or, for Boost Protect with AppleCare Services, the difference between the malfunction repair deductible and all other damage repair deductible if the phone has damage.

How does unlimited claims for screen break repairs and accidental damage work?

With Boost Protect and Boost Protect with AppleCare Services, you are eligible for unlimited claims for screen break repair at a $29 deductible each as well as unlimited claims for all other damage repair at a $99 deductible each.

* With Boost Protect, malfunction is covered after the expiration of the original manufacturer’s warranty. A manufacturer's warranty is provided by your phone manufacturer and is generally valid for 12 months from the date of purchase. How long do I have to file a claim?

If my device is replaced or repaired, am I protected for the new device?

Boost Protect
Replacement phones come with a 90-day warranty. In the event the device malfunctions during this 90-day warranty period, we will repair or replace it at no cost to you. Repairs come with a 90-day warranty on the repair performed. If you experience issues with the repair during this time, we will repair or replace your phone at no cost to you.

Boost Protect with AppleCare Services
Your replacement phone is covered against malfunction for as long as you are enrolled in the program. If you exit the program, your warranty with Apple is the remainder of the 12-month warranty or 90 days, whichever is greater. Repairs come with a 90-day warranty on the repair performed. If you experience issues with the repair during this time, we will repair or replace your phone at no cost to you.

If you are having issues with your device during this period, please follow the existing claim process to report the problem.

Will I get the same device for an approved claim if my phone is lost, stolen, or unrepairable?

Boost Protect with AppleCare Services
Your device will be replaced with a new or Apple certified device of the same make, model, color and memory capacity.

Boost Protect
Your device will be replaced with a wireless device of like kind and quality with comparable features and functionality and may be new or reconditioned.

Am I required to pay a deductible before I get my device repaired or replaced?

Yes. You are required to pay a non-refundable deductible for each approved claim. The exception to this is if you are enrolled in Boost Protect with AppleCare Services and your device malfunctions. Warranty malfunction claims with Apple have a $0 deductible as long as there is no physical damage to the device.

How will I be billed for my monthly fee?

Your Monthly Fee will be added to your monthly Boost Mobile bill.

How do I cancel coverage?

You may cancel your coverage at any time by contacting Boost Mobile Customer Care at 1-833-502-6678.

What should I do with my damaged device?

If you receive a replacement device due to an approved accidental damage from handling or malfunction claim, you’ll need to return your broken device to Likewize or Apple. Shipping instructions and shipping label for returning the damaged equipment will be included with the replacement device. Be sure to act quickly! Failure to do so within 10 days will result in a non-returned equipment fee up to the MSRP of your device at the time of enrollment.

What determines which repair options are offered?

Your location, device model, type of damage, and repair or replacement part availability can influence your repair choices. For example, brand new models may not be available for repair right away, and certain types of damage such as liquid damage are not eligible for repair.

Can I go to any phone repair location?

To ensure quality, all repairs must be completed through an authorized repair partner provided and chosen at the time you file your claim.

What if there’s not a repair location near me, but I don’t want to replace my device?

Don’t worry, we have repair partners who accept devices via mail. Likewize or Apple will provide a prepaid label. Just package your device and drop it in the appropriate mailbox. In most cases, it will be repaired within 3-5 business days from receipt and returned to you via next business day delivery.

What happens if the repair provider cannot fix my device?

If your device is unrepairable, you will receive a replacement.

What happens if I have an issue with my device after repair?

Repaired devices come with a 90-day warranty for the repair made. If you have an issue with the repair during this time, we will repair or replace your device at no cost to you. Just follow the existing claim process to report the problem.

What security features are included with my Boost Protect and Boost Protect with AppleCare Services program?

Boost Protect and Boost Protect with AppleCare Services subscribers receive the Lookout® Premium mobile security app at no extra cost as part of your protection program.

Lookout® Premium proactively protects your devices and digital information from threats, automatically keeps devices and apps up to date with the latest security protection, and scans downloads for anything suspicious – all in one easy to use app.

Will the Lookout® Premium Mobile Security work on my phone?

    As of September 1, 2022, if you previously purchased a Sprint/United Management device protection policy (5471000 or 5471003), your new policy number is 5470995 and 5471001. However, you do not need your policy number to file a claim.
